When is PCNL Recommended?
PCNL is recommended in the following conditions:

How PCNL is done
- Under anaesthesia, the kidney is punctured through the back under X-ray or ultrasound guidance.
- The track is widened to allow a telescope into the kidney.
- The stone is broken with a laser or other energy and the fragments are removed.
- A stent or small drainage tube may be left for a short time.
Smaller-track versions, Mini-PERC and Micro-PERC, are used for medium-size stones to reduce bleeding and pain.

- Hard stones that did not break with ESWL

Recovery and possible risks
Most patients walk the next day and go home in two to three days, returning to light work in about a week. Blood in the urine for a day or two is common. Less common risks include bleeding needing treatment, infection and, rarely, injury to nearby organs; choosing an experienced endourologist and careful planning with a CT scan keep these low.

Is PCNL a major operation?
It is a keyhole operation through a cut of about 1 cm, not open surgery. It does need a short hospital stay of usually two to three days.
What size of kidney stone needs PCNL?
Generally stones over about 2 cm or staghorn stones. Medium stones may be treated with RIRS or Mini-PERC instead.
Will I have a tube after PCNL?
Some patients have a small drainage tube for a day or two, or a stent inside instead (tubeless PCNL). This depends on how the surgery goes.
