What is a short frenulum?
The frenulum is the small band of skin on the underside of the penis that connects the foreskin to the head (glans). When it is short or tight (frenulum breve), it can pull the head downwards during erection, cause pain or bleeding during intercourse, and tear repeatedly. Each tear heals with scar tissue that makes the frenulum even tighter.
Symptoms of a tight frenulum
- Pain or a pulling sensation during erection or intercourse
- Bleeding or tearing of the frenulum during sex
- Downward bending of the head of the penis during erection
- Difficulty pulling back the foreskin fully
- Anxiety about intercourse, sometimes with premature ejaculation
What is laser frenuloplasty?
Frenuloplasty is a small operation that lengthens the frenulum so it no longer pulls. In laser frenuloplasty, a precise laser is used to release the tight band, with minimal bleeding and swelling; the wound is then shaped so it heals with a longer, more elastic frenulum. It treats the cause of pain and tearing while keeping the foreskin.
The procedure
- Anaesthesia: usually local anaesthesia; the procedure is painless.
- Release: the laser releases the tight band across its width.
- Closure: the wound is closed lengthwise with fine dissolvable stitches, which lengthens the frenulum.
- Home: a day-care procedure; most men go home within a couple of hours.
Benefits of laser frenuloplasty
- Short, day-care procedure under local anaesthesia
- Minimal bleeding and swelling with the laser
- Keeps the foreskin (no circumcision needed when the foreskin is otherwise normal)
- Ends painful tearing and improves comfort during intercourse
- Quick return to work, usually the next day
Recovery and aftercare
| When | What to expect / do |
|---|---|
| Day 1–3 | Mild soreness and swelling; simple painkillers; keep the area clean and dry |
| Week 1–2 | Stitches dissolve; gentle washing; loose cotton underwear |
| About 4 weeks | Avoid intercourse and masturbation until fully healed, usually around four weeks |
Contact the clinic if there is bleeding that does not stop with gentle pressure, increasing pain, pus or fever.
Frenuloplasty or circumcision?
If the only problem is a short frenulum and the foreskin retracts normally, frenuloplasty keeps the foreskin and is a smaller procedure. If the foreskin is also tight (phimosis), scarred, or infections keep recurring, ZSR circumcision may be the better solution, and the frenulum can be corrected at the same time. Read about tight foreskin.
Laser or conventional frenuloplasty?
Both lengthen the frenulum. The laser seals small blood vessels as it works, which usually means less bleeding and swelling and a neat result. The choice of technique is discussed at consultation.
Questions men often ask
- Does a short frenulum cause premature ejaculation? It can contribute, because pain and anxiety affect control. Some men find control improves after frenuloplasty.
- Will it affect sensation? Sensation is normally preserved; many men report more comfortable intercourse.
- Is it embarrassing to ask about? Not at all. It is a common problem, and the consultation is private.
How a short frenulum is diagnosed
Diagnosis is made by a simple, private examination and a discussion of your symptoms. The urologist checks the length of the frenulum, whether the foreskin retracts fully, and for any scarring from earlier tears, infection or phimosis. No special tests are usually needed. If there is a history of infections, a urine test or a blood sugar test may be advised.
Can a short frenulum be treated without surgery?
Mild cases sometimes improve with gentle stretching and using plenty of lubrication during intercourse. However, if the frenulum keeps tearing, bleeds, or causes pain or bending of the penis, stretching rarely solves it, and repeated tears make the band tighter with scar tissue. Frenuloplasty corrects the problem in one short procedure.
If your frenulum has torn
A torn frenulum often bleeds a lot because it contains a small artery. Press firmly on the area with a clean cloth for 10–15 minutes. Most tears stop bleeding with pressure and heal in one to two weeks; avoid sex until it has healed. If bleeding does not stop, seek medical help. Once healed, see a urologist about frenuloplasty to prevent it tearing again.

Frequently asked questions
Is laser frenuloplasty painful?
It is done under local anaesthesia, so the procedure itself is not painful. Mild soreness for a few days is controlled with simple painkillers.
How long does laser frenuloplasty take?
Usually 20–30 minutes, and you go home the same day.
When can I have sex after frenuloplasty?
Usually after about four weeks, once the wound has fully healed.
Is frenuloplasty the same as circumcision?
No. Frenuloplasty only lengthens the frenulum and keeps the foreskin. Circumcision removes the foreskin and is advised when the foreskin itself is tight, scarred or repeatedly infected.
Will frenuloplasty change how my penis looks?
The appearance hardly changes. The frenulum is made longer, and the scar is small and on the underside, so it is barely noticeable once healed.
Can a short frenulum cause the penis to bend?
Yes. A short frenulum can pull the head of the penis downwards during erection. This differs from Peyronie’s disease, in which the shaft itself bends because of a scar inside the penis.
How do I wash after frenuloplasty?
Follow the advice given at discharge; usually you can shower after a day or two, gently wash with plain water, and pat the area dry. Avoid soaking in a bath or swimming until it heals.
Do I need someone to drive me home?
If the procedure is done under local anaesthesia only, most men can travel home normally. If sedation is given, someone should accompany you.
Is frenuloplasty covered by insurance?
When it is done for a medical problem such as pain or repeated tearing, many policies cover it. The clinic provides an estimate at consultation.
Will the tear come back after frenuloplasty?
Frenuloplasty lengthens the frenulum so it no longer pulls, which usually stops repeated tearing.
